I acknowledge the following:
- This is group consultation and therefore the time is shared between attendees
- The agreed-upon fee per group session is $60
- Each group is limited to 4 attendees
- Cancellation policy:
- Cancelations up to 2 weeks prior—may be refunded
Cancelations up to 1 week prior—may be rescheduled for another available date
Cancelations less than a week prior will be forfeited (except in emergent circumstances)
- Cancelations up to 2 weeks prior—may be refunded
- Dr. Muhlheim is considered a consultant and is not legally responsible or liable for attendees’ patients.
- Attendees are responsible for keeping up with their own Ethical Principles and Code of Conduct, laws, and regulations
- This does not count for certification in FBT
- This may count towards CEDS certification
- Meetings will be over Zoom and Dr. Muhlheim is not responsible for connectivity issues
- Identifying information of clients should be protected
